Create and publish WordPress forms

WPZOOM Forms PRO extends a drag-and-drop form builder for creating contact, feedback, appointment, booking, quote, job application, and support forms without writing code. Build forms in a dedicated section of the WordPress dashboard, where you can arrange fields and layouts, start from a template, or describe what you need in plain language for the AI Form Generator to configure.

When a form is ready, publish it with the WPZOOM Form block, the dedicated Elementor widget, or a shortcode. Shortcodes also work with themes, page builders, widget areas, and the Classic Editor. Forms are mobile responsive.

PRO fields and form settings

• AI Form Generator: Describe a form in plain language and generate a configured form.
• Conditional logic: Show or hide fields based on earlier answers.
• File uploads: Accept PDF documents and pictures, such as resumes, cover letters, project documents, portfolios, and support attachments.
• Date and time pickers: Use date fields for bookings, reservations, and appointments. The time picker works with 12-hour or 24-hour formats and configurable minute intervals.
• Number fields: Collect numeric entries with validation.
• GDPR consent fields: Add a consent checkbox with custom text and a link to your privacy policy.
• Hidden fields: Capture UTM parameters, referrer data, and other metadata.

Connect submissions and manage follow-up

Mailchimp integration can add submissions to an audience for newsletter signups or other email marketing lists. AJAX submissions display confirmation messages without a full page reload. A visual editor lets you personalize notification email layouts and add dynamic field tags.

Submissions are stored in the WordPress dashboard, where you can search and filter entries. Export submissions as CSV, and use JSON import and export to transfer forms or make backups. You can also duplicate an existing form to make a variation.

Templates and spam protection

Ready-to-use templates cover contact, feedback, quote requests, appointments, leads, waitlists, demos, audits, events, support, education, real estate, restaurants, and local services. The PRO template library includes more than 20 industry-specific layouts, such as business consultation, project inquiry, patient intake, job application, and restaurant reservation forms.

The form builder also includes spam protection options for Google reCAPTCHA v2 and v3, Akismet, and Cloudflare Turnstile.

Set up a form

1. Install WPZOOM Forms PRO in WordPress and open its dashboard section.
2. Create a form with the drag-and-drop builder, select a template, or use the AI Form Generator.
3. Configure the fields and any needed PRO options, such as Mailchimp, uploads, conditional logic, or custom email notifications.
4. Publish the form with a block, Elementor widget, or shortcode.
