Premium Form Maker by 10Web is a drag-and-drop WordPress form builder for collecting contact requests, feedback, survey responses, applications, registrations, orders, and other information from website visitors. Forms are designed for mobile, tablet, and desktop resolutions, and customizable themes let you adjust their appearance to the surrounding website.

Build fields and form flow

Assemble simple or complex forms from 43 field types, or start with one of five pre-built templates. Choose from 15 customizable themes. Conditional logic can display or use form components according to submitted information, while multi-page layouts divide longer forms into separate pages.

Publish forms and manage entries

Embed a form in a blog post or show it as a popup, top bar, or scroll box. Submission management lets you track and export entries and configure automatic email replies. Spam protection options include blocking IP addresses and setting captchas.

Payments, file uploads, and extensions

The Premium version includes payment and donation collection through integrated PayPal and Stripe gateways. It also includes file uploads, so visitors can attach documents to submissions.
